The typed application should include a completed cover sheet, proposal information and supporting materials listed below.

Proposed Information
1. General information about the organization initiating the funding request, including history and programs offered. A brochure may be included if one is available.
2. Description of the program or project for which funds are requested, a proposed budget for the Project and how it aligns with MAD’s mission.
3. Description of the need, who will benefit, and what other organizations or programs exist in the area that address similar needs.
4. Statement of how the program or project will be implemented, including chronology of activities and dates for implementation and conclusion.
5. Amount of the request and timing of disbursement.
6. Summary of the qualifications of the staff or individuals administering the project or program.
Required Supporting Materials
1. a 501(c)(3) or a 501(c)(13) entity, a copy of the latest determination letter issued by the Internal Revenue Service regarding tax exempt status under Section 501(c)(3) or Section 501(c)(13); a statement that no change has occurred in the tax-exempt status or function of the organization since the latest ruling; and a complete copy of IRS form 990 including schedules for the most recent fiscal year.
2. The most recent financial statements of the applicant, audited if available. This statement should reflect actual expenditures and funds received during the most recent fiscal year.
3. Overall organizational budget of the applicant for the year in which the funding is requested.
4. A current list of the applicant’s board of directors or trustees.
Supplement Supporting Materials
Copies of news articles, photographs, and video or audio tapes relating to the organization and/or project or program, letters of support from outside sources, or other pertinent information.
•The Foundation board of directors considers requests for funding annually. Applications should be submitted on or before May 31st for consideration in July. The Foundation may also fund projects outside the regular funding cycle when alternative timing is deemed critical to an organization making a request.
•Site visits will be initiated by the board, if needed, and direct solicitation of the board is not encouraged.
•All applicants will be notified of the action taken by the board of directors on their requests.
•All materials submitted become property of the MAD Foundation.
